He, Ben and Richard split from the group and episode or two back. I forget where they were going other than *not* toward Smokey. This is what happens when you break for a week and then not bother with previouslies when you come back.
And as far as I'm concerned, Frank swam out of the sub and went back to the other island to chill with Rose, Bernard and Vincent. Yep, that's totally what happened.

Hee, yes, I noticed that as well. Maybe you wouldn't bleed to death from a shoulder through-and-through shot but I'm no expert, unlike the Lost writers. ;)
